Certified Nurse Aides are licensed and are able to provide a higher skill level of personal care needs, for those with greater physical limitations. This could also include bathing, skilled transfers, skin care, ostomy care, checking vital signs, assisting with range of motion exercises or ambulation exercises, incontinence care, and feeding.
The enrollee receives both Medicaid and Medicare services from one plan. Once your doctor refers you for home health services, the home health agency will schedule an appointment and come to your home to talk to you about your needs and ask you some questions about your health. The home health agency staff will also talk to your doctor about your care and keep your doctor updated about your progress. It’s important that home health staff see you as often as the doctor ordered. Home is the most comfortable and comforting place to heal when you are dealing with a health challenge.

Our Services
This position is accountable for the start of care to admit new clients to home health. This includes coordination of in-home nursing care, including direct client care, patient/family education and transitions of care. Must evaluate client nursing needs and develop plans of care and goals appropriate to client assessment per physician orders.
